  • Primary Care Services (VI) Additional Appointments Available

    Appointments are now available on Saturdays, Sundays and Bank Holidays for patients of practices within Primary Care Services (VI).

    Appointments will be delivered from West Park Surgery West Park Hospital between 8am and 2pm. 

    Appointments will be available on a first-come first-serve basis to be booked in prior to the day through your normal GP practice.

    If you need to book an urgent appointment please call 111 who will redirect your call; alternatively you can call the practice directly on 01902 422677 on the day.

    Primary Care Services (VI) have also Improved Access during the week with weekday appointments available from 5pm – 8pm. For further information please contact your practice team.

ON-LINE SERVICES

There are a number of electronic service developments affecting GP practices which are being made available to our patients, some of which are already operating.

Practice Statement of Intent

PATIENT ONLINE SERVICES

It is a contractual requirement for GP practices to offer and promote to patients: online booking of appointments, ordering of repeat prescriptions and by 31st of March 2015 access to summary information (as a minimum) in their patient record.

PATIENT ACCESS TO THEIR GP RECORD

Practices are required to promote and offer the facility to view online, export or print any summary of information from their records relating to medications, allergies, adverse reactions and any other items such as ‘additional’ record elements which has been agreed between the contractor and the patient, or have published plans in place to achieve this by 31st March 2015.

The practice currently meets this requirement.

REFERRAL MANAGEMENT

All practices must include the NHS Number as the primary identifier in all NHS clinical correspondence issued by the practice.

ELECTRONIC APPOINTMENT BOOKING

Practices are required to promote and offer the facility for all patients, who wish, to book, view, amend, cancel and print appointments online.

The Practice currently offers this facility. If you do not already have a user name and password for this service, please speak to a member of the reception team.

ONLINE REPEAT PRESCRIPTION REQUESTING

Practices are required to promote and offer the facility for all patients, who wish to order online, view and print a list of their repeat prescriptions for necessary drugs, medicines or appliances.

The practice currently offers this facility. If you do not already have a user name and password for viewing and printing a list of your repeat medications, please speak to a member of the reception team.

SUMMARY CARE RECORD

Practices are required to enable successful automated uploads of any changes to a patient’s summary information, at least on a daily basis to the Summary Care record, or have published plans in place to achieve this by 31st March 2015.

A Summary Care Record (SCR) is a brief summary of your medical records that will help anyone treating you in an emergency; where they do not have your full medical record. They will have access to information about any medication you may be taking and any drugs that you have a recorded allergy or sensitivity to. Your SCR is automatically updated at least daily to ensure that your information is as up to date as it can possibly be.

If you do not want your medical records to be available in this way then you will need to let us know so that we can update your record. You can do this by speaking to a member of the reception team.

GP2GP RECORD TRANSFERS

There is a contractual requirement to utilise the GP2GP facility for the transfer of patient’s records between practices, when a patient registers or de-registers.
It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England. It can take several weeks for your paper records to reach your new surgery. With GP to GP record transfers, your electronic record is transferred to your new practice much sooner.

Register for Online services

In order to access the online services available you will need to attend the practice in person with photographic identification e.g. photo driving licence or passport to register for this service.

Once we have verified your identity you will be given your unique identifier number. This will enable you to create an account so you can start to use the facilities available. Please speak to a member of the reception team for further information.
