  • Primary Care Services (VI) Additional Appointments Available

    Appointments are now available on Saturdays, Sundays and Bank Holidays for patients of practices within Primary Care Services (VI).

    Appointments will be delivered from West Park Surgery West Park Hospital between 8am and 2pm. 

    Appointments will be available on a first-come first-serve basis to be booked in prior to the day through your normal GP practice.

    If you need to book an urgent appointment please call 111 who will redirect your call; alternatively you can call the practice directly on 01902 422677 on the day.

    Primary Care Services (VI) have also Improved Access during the week with weekday appointments available from 5pm – 8pm. For further information please contact your practice team.

Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. You can use the link below to print off the questionnaire which you may fill out and bring with you when you come to the surgery. You will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
